Consent and communication disclosure

The consent you give when you check the consent box and submit the evaluation.

Effective date: June 11, 2026

This page explains the consent you give when you check the consent box and submit the evaluation on this website.

What you are agreeing to

By checking the consent box and clicking the submit button, you provide your express written consent to be contacted by our marketing partners (a current list is available upon request at support@qualifyfordisability.com) at the telephone number and email address you provided, regarding disability benefits services and your evaluation.

How you may be contacted

Contact may include live calls, calls using an automatic telephone dialing system, prerecorded or artificial voice messages, text messages (SMS/MMS), and emails. Message and data rates may apply. Message frequency varies.

Important points

Your consent is not a condition of purchasing any goods or services. You are not required to consent to receive help with disability benefits; you can contact the Social Security Administration directly at ssa.gov or your local office. Providing a number means you represent you are the subscriber or customary user of that number.

How to revoke consent

You can revoke consent at any time by replying STOP to any text message, telling any caller to stop calling, or emailing support@qualifyfordisability.com with the number or email you want removed. Revoking consent with one partner does not automatically revoke it with others; we will assist you in stopping contact from any partner who received your information through this site upon request.

Record of consent

We capture and retain proof of consent for each submission, including the consent language shown, the date and time, the page, and independent certification (such as TrustedForm).
